How to Report to OSHA
- Call the nearest OSHA office.
- Call the OSHA 24-hour hotline at 1-800-321-6742 (OSHA).
- Report online: Report a Fatality or Severe Injury | Occupational Safety and Health Administration (osha.gov)
- Be prepared to supply your business name; names of employees affected; location and time of the incident, brief description of the incident; contact person, email and phone number.

Recordkeeping
- Tree Service Companies with more than 10 employees are required to keep a record of serious work-related injuries and illnesses
- Recording serious work-related injuries requires the use of an OSHA 300, 300-A and 301 (or equivalent). Link to forms: https://www.osha.gov/index.php/recordkeeping/forms

Submitting Injury and Illness Data
- Establishments with 250 or more employees that are currently required to keep OSHA injury and illness records, and establishments with 20-249 employees that are classified in certain industries with historically high rates of occupational injuries and illnesses.
- Establishments in the following industries with 20 to 249 employees must submit injury and illness summary (Form 300A) data to OSHA electronically.
- Due annually by March 2. Covered establishments must submit information from their completed Form 300A.
